OAK BROOK, Ill. Based on a study of nearly 2,000 breast cancer patients, researchers at the Swedish Cancer Institute in Seattle say that, in women between the ages of 40 and 49, breast cancers detected by mammography have a better prognosis. The study appears in the March issue of Radiology.
"In our study, women aged 40 to 49 whose breast cancer was detected by mammography were easier to treat and had less recurring disease and mortality, because their cancer was found at an earlier stage," said Judith A. Malmgren, Ph.D., president of HealthStat Consulting, Inc.
Dr. Malmgren and a team of researchers reviewed breast cancer patient data from a dedicated registry at the Swedish Cancer Institute's community cancer center. The researchers analyzed data on 1,977 breast cancer patients between the ages of 40 and 49 who were treated between 1990 and 2008. The researchers looked at method of diagnosis (detected by mammography, patient or physician), stage at diagnosis (0-IV, confirmed by biopsy), treatment, and annual follow-up information, including recurrence of disease.
"Our goal was to assess the differences between mammography and non-mammography detected breast cancer, to determine whether earlier detection confers a treatment and morbidity advantage because the disease is found at an earlier stage," Dr. Malmgren said.
The data analysis revealed a significant increase in the percentage of mammography-detected breast cancer over the 18-year period: from 28 percent in 1990 to 58 percent in 2008. Over the same period, patient- and physician-detected breast cancer declined from 73 percent of all cases in 1990 to 42 percent in 2008.
"The shift toward more mammography-detected breast cancer cases was accompanied by a shift toward diagnosis at an earlier stage of disease that required less treatment," Dr. Malmgren said.
Over the 18-year period, the number of breast cancers diagnosed at Stage 0 increased by 66 percent, while the number of Stage III breast cancers decreased by 66 percent. The majority of Stage 0 cancer cases were ductal carcinoma in situ (DCIS), a non-invasive cancer that is confined to a milk duct. The treatment of DCIS remains controversial, because not all experts agree that it is potentially life threatening.
Dr. Malmgren said another key finding of the study was the extent of treatment patients received. Compared with women whose cancer was self-detected or discovered by a physician, patients whose cancer was detected using mammography were more likely to have breast-conserving treatment and less likely to have chemotherapy. Specifically, they were more likely to undergo lumpectomy (67 percent versus 48 percent), less likely to undergo modified radical mastectomy (25 percent versus 47 percent), and less likely to die of breast cancer (4 percent versus 11 percent).
"The benefits of breast cancer treatment are accompanied by significant harms," Dr. Malmgren said. "Chemotherapy may have long-lasting toxic effects on a woman's body, and mastectomy and reconstructive surgery are difficult and expensive operations that can have a significant effect on body image."
The American Cancer Society and other medical organizations recommend that women should begin receiving annual mammograms to screen for breast cancer beginning at age 40. In 2009, the U.S. Preventive Services Task Force issued controversial new guidelines that eliminated the longstanding recommendation for routine mammography screening for women between the ages of 40 and 49, stating that the benefits of screening were potentially outweighed by the harms of screening, such as false-positive results.
"The objective of screening is to detect disease at an earlier, more treatable stage, whichbased on our reviewmammography accomplishes," Dr. Malmgren said.

LOS ANGELES (AP) ? A judge granted a motion Tuesday to dismiss concert promoter AEG Live from a wrongful-death lawsuit filed by Michael Jackson's father.
City News Service said Superior Court Judge Yvette Palazuelos agreed with AEG attorneys who said Joe Jackson should have joined a separate suit filed by his wife, Katherine. The judge said the law does not favor multiple suits by individual heirs.
She said that to allow Joe Jackson's suit against AEG Live to go forward would "allow the heirs of a decedent to file as many suits as there are heirs."
"Allowing Joseph Jackson to continue with his subsequent suit against AEG Live, alleging the same wrongful death claims as the related case, would contravene the purpose of the one-action rule," she said.
AEG also argued that Joe Jackson is not a legitimate heir to his son's fortune. The father was not mentioned in the King of Pop's will.
"At the time Michael Jackson died, (his father) was estranged from his son, who repeatedly and publicly accused him of physical and other abuse," the AEG Live court papers said.
The decision leaves Dr. Conrad Murray and Applied Pharmacy Services of Las Vegas as the only defendants in Joe Jackson's suit. Murray is in jail after having been convicted of involuntary manslaughter in Michael Jackson's death. He says he is broke.
Katherine Jackson alleges in her suit that AEG Live is responsible for medical decisions made by Murray.
AEG Live attorney Marvin Putnam called the ruling a major victory for the entertainment giant. Joe Jackson's lawyer Maureen Jaroscak called it a temporary setback and suggested that the 83-year-old Jackson might now try to become a party to his wife's lawsuit.
Michael Jackson died in June 2009 from an overdose of the anesthetic propofol, which Murray was administering to him as a sleep medication.

The debris travels at speeds approaching 17,500 miles per hour (28,000 kilometers per hour), fast enough to destroy or inflict costly and time-draining damage on a satellite or spacecraft. Collisions, in turn, generate more fragments floating in space.
"It has become essential to be aware of the existence of this debris and the risks that are run by its proliferation," said Claude Nicollier, an astronaut and EPFL professor.
Building the satellite means developing new technology to address three big problems, scientists say.
The first hurdle has to do with trajectory: The satellite has to be able to adjust its path to match that of its target. EPFL said its labs are looking into a new ultra-compact motor that can do this.
Next, the satellite has to be grab hold of and stabilize the debris at high speeds. Scientists are studying how plants and animals grip things as a model for what would be used.
And, finally, CleanSpace One has to be able to take the debris, or unwanted satellites, back into Earth's atmosphere, where they will burn on re-entry.

More men said that they had had a same sex sexual experience too, up from 5.3% in 1990 to 8.4% in 2000, thus showing that either same sex sexual behaviour is on the increase or people are more willing to report it.
When looking at same sex sexual attraction, and not necessarily sexual experience, the figures have also changed over the ten-year period, with women showing the most significant difference. In 1990, 93.3% of men said they had only ever had sexual attraction towards the opposite sex, whilst by 2000 this had fallen to 91.9%. 93.6% of women in 1990 said they had only ever been attracted to men, but by 2000 this had dropped to 88.3%. From this we can therefore deduce that 11.7% of women and 8.1% of men have felt a sexual attraction towards the same sex at least once in their lives.2?3Among?men?they found the following results:

More recently, data has been collected in America, during the ten-year national census, on married and unmarried-partner households. They did not ask the actual sexual orientation of the respondents, so there is no measure of single gay people, nor is there a measure of those gay people in committed relationships but not living together. Whilst the census cannot give us a figure for the number of people who are gay in America, it can inform us on how many same-sex partnership households identified themselves in the survey. The 2000 census tells us that
There have been various other surveys in the US that have tried to measure numbers of gay people. An analysis of these surveys by the Human Rights Campaign came up with this conclusion:
"In the last three elections, the Voter News Service exit poll registered the gay vote between 4 percent and 5 percent. While concluding that the Census 2000 undercounted the total number of gay or lesbian households, for the purposes of this study, we estimate the gay and lesbian population at 5 percent of the total U.S. population over 18 years of age, (209,128,094). This results in an estimated total gay and lesbian population of 10,456,405. A recent study of gay and lesbian voting habits conducted by Harris Interactive determined that 30 percent of gay and lesbian people are living in a committed relationship in the same residence. Using that figure, we suggest that 3,136,921 gay or lesbian people are living in the United States in committed relationships in the same residence."7
So, if we accept that the data presented by the Human Rights Commission is indeed indicative of the real numbers, then it shows that the census data is only showing up a small percentage of the actual number of gay people living in America. Until, however, a nationwide survey is done, asking questions on sexual attitudes and behaviour, then we can only use data and analyses, such as the ones above, to work out an answer to the question of gay people living in America.
Very little research has been carried out into the frequency of same-sex behaviour in non-Western countries. However, the studies carried out in the US and the UK reveal a diversity of sexual identities, which it is reasonable to presume are equally prevalent throughout the world. Widespread stigmatisation and criminalisation of same-sex behaviour in many countries prevent gay men and lesbians from openly expressing their sexuality. This has led to an invisibility of gay identities. This does not mean that gay people do not exist in these places, but in fact highlights the importance of increasing recognition of gay communities - something that is important in its own right, but also crucial for HIV prevention.
In Africa, for example, political and cultural leaders have consistently denied the existence of homosexuality in their communities. In 2004, the President of Nigeria declared that homosexuality ?is clearly un-Biblical, un-natural, and definitely un-African?.8?However, gay and lesbian Africans have become increasingly vocal as global movements have opened up public discussions about sexuality and gender. Consequently, a significant minority of self-identified gay men and lesbians has emerged throughout the continent.9 The Kenyan government announced plans in 2009 to carry out a nationwide survey to gain a better understanding of the numbers of gay people living in the country.10Such a survey would be the first of its kind to be carried out in Africa, and provide important information for HIV prevention. However, Kenya's plans also highlight the barriers to establishing such estimates.
